All D'Youville students are required to submit a health form and immunization records through our partner CastleBranch.
In an effort to make submitting and maintaining your records simple, D'Youville has partnered with CastleBranch to manage all required immunization records. Once you submit your records, you'll want to retain your login credentials so you can access your account throughout your studies, from updating your records to adding new records required for clinical rotations.
A pre-entrance physical examination is not required, but is recommended for your personal records. All students must meet New York State immunization requirements.
To submit your health records, simply visit the CastleBranch website, create an account, and click "Place an Order." Once you place your order, you will be asked for a PIN, which is your full Student ID with the “00” in the beginning.
Remember to retain your CastleBranch credentials for future use as additional vaccination records may be required for clinical rotations and field work.
Should your program require field work, you will be contacted by your department with instructions on how to submit any additional records. Be sure to retain your login credentials, as you'll use those to update your records when you begin your fieldwork.
