D’Youville Will Offer COVID-19 Vaccines Starting April 30

Buffalo, New York – April, 2021 – The D’Youville School of Pharmacy is excited to announce it will begin offering COVID-19 vaccinations to students, faculty, staff, alumni, and community members age 18 and up starting Friday, April 30.

Free vaccinations using the Moderna vaccine will begin on April 30, from 11 a.m. to 7 p.m., and will take place on the first floor of the D’Youville Academic Center, located at 242 Fargo Ave. in Buffalo. Times and dates for further clinics will be announced shortly.

Appointments are required and can be made at go.dyc.edu/vaccine. Once an appointment is confirmed, patients must complete the online New York State COVID-19 Vaccine Form at forms.ny.gov/s3/vaccine and bring the submission identification or confirmation email to their appointment.

The Moderna vaccine is a two-dose series administered 28 days apart into the muscle of the arm for patients ages 18 and up. When a patient presents to receive their first dose, the D’Youville clinic team will schedule the second dose 28 days later for the same time of day.

Patients should arrive 10 minutes prior to their appointment. Photo identification, insurance card and mask are required. There is no cost. Consent and registration forms must be filled out at the appointment and patients must remain on site for at least 15 minutes after receiving the vaccine for required monitoring of any potential reactions.
