PGY1 Community-Based Pharmacy Residency Program

Vital Pharmacy resides within D’Youville’s Health Professions Hub on the D’Youville University campus—a nexus where various health professions collaborate, learn, and care for the community together. The hub encompasses Vital Pharmacy, Sisters Health Center D’Youville, D’Lish dietetics demonstration kitchen, and Thrive physical therapy, occupational therapy, dietetics, and chiropractic clinic, among others.

Orientation to Residency and Patient-Centered Dispensing:

The purpose is to orient residents to the expectations of each core rotation, the residency program, the site, and any applicable technology. Residents will be oriented to the residency manual; purpose; ASHP Standard; competency areas, goals, and objectives; description of learning experiences; complete organization new hire orientation (including reporting harassment and inappropriate behavior); and discuss strategies for maintaining well-being and resilience with resources. Residents will review residency policies within 14 days of program start and document this per the Orientation Checklist.

Patient-Centered Dispensing:

Includes patient-centered medication distribution, point-of-care testing, immunizations, use of protocols (i.e., naloxone, hormonal contraceptives, PrEP, etc.), and administration of medications as applicable, among other activities.

Clinical Programs and Direct Patient Care:

Includes comprehensive and targeted medication management, patient care consults for protocols (i.e., naloxone, hormonal contraceptives, PrEP, etc.), chronic condition management (diabetes, hypertension, dyslipidemia, asthma, COPD, heart failure, arthritis, pain management, Alzheimer's disease, end-stage renal disease, mental health, etc.), educational programs (regarding medications, medication therapy management, and health-related topics), disease prevention and wellness activities (tobacco cessation, weight loss, naloxone education, health screenings, risk assessment, medication take-back, disease prevention education, etc.) and any applicable physical assessments (I.e., weight or blood pressure for DSMES classes), as well as transitions of care medication reconciliation and management. Residents will gain experience in care for a variety of disease states and range of medications and health-related needs; recurring follow-up of patients assigned to develop care continuity and extended patient relationships; and communication of care with other members of the healthcare team and patient/caregiver.

Leadership:

Includes engagement in the pharmacy population and population served; self-assessments for quarterly development planning; practice demonstrating personal and interpersonal skills to manage responsibilities and practice demonstrating professional behavior and responsibilities; learning series/topic discussions on factors that influence pharmacy needs and planning; participation in administration formal and/or informal discussions on external factors that influence pharmacy in the healthcare environment.

“Eliana is one of the current pharmacy residents for the 2026–2027 academic year. She earned her Bachelor of Science from the University of Waterloo before completing her Doctor of Pharmacy at D’Youville University.

Her residency research project, “Pharmacist-Led Medication Reconciliation and Adherence Packaging Initiation for Patients Transitioning from Sub-Acute Healthcare Facilities Back into The Community,” focuses on improving medication management and supporting patients during transitions of care.

Eliana is interested in addressing social determinants of health and their impact on medication access and health outcomes. Similarly, she is passionate about working with older adults and assists with Vital Pharmacy's Age-Friendly medication reviews (Vital Connections) that allows patients to communicate their concerns in managing their health.

Her other professional interests include academia and ambulatory care. In her free time, Eliana enjoys reading!"

Kaylee is one of the current pharmacy residents for the 2026-2027 academic year. She earned her Doctor of Pharmacy degree from the University at Buffalo in 2026. Her residency research project, Bridging the Gap in Diabetes Technology Through Pharmacist-Led CGM and Omnipod Initiation, reflects both her professional interests and personal experiences. As an individual living with type 1 diabetes, Kaylee is passionate about improving access to diabetes technology and advancing chronic disease state management. Her career interests include academia and ambulatory care. In her free time, Kaylee enjoys skiing and playing pickleball.
